Name Change Information

Flushing High School Yearbook has a desire to create a book for every student in our building.  We understand that sometimes a name change is required to help all students feel validated and involved.  If a name change is desired, we have a formal policy in place and a form that must be completed.  Please note that in order for there to be an edited name change within the yearbook, everything below must be followed and submitted.
